Completing this form will enable you to directly refer for Basic Telecare which consists of:

A base unit which is connected to your telephone line and a nearby electrical socket

A small waterproof red button. Used to call for help

A smoke detector which is linked to the 24/7 monitoring centre

If you would like to refer for enhanced telecare (bed sensors, door sensors etc) please contact your local Single Point of Access.

You will not be able to complete the telecare referral form if you are not eligible for the service, so please check your eligibility before you begin. To be eligible you must have:

  • a working landline telephone (capable of receiving incoming and making outgoing calls)
  • a spare electrical socket within 1 metre of your telephone socket (that won’t cause wires to trail across your floor)
  • preferably 3 responders, but at minimum 2 responders, who live at different adresses and can attend your property within 30 minutes
